Drowning Prevention Australia Incident Investigation in Aquatic Facilities

When an incident occurs in an aquatic facility, understanding what happened is only the beginning. A thorough investigation can uncover the underlying factors that contributed to an incident and, importantly, identify opportunities to strengthen systems and help prevent it from happening again.

Drowning Prevention Australia is pleased to introduce a new professional development course through the DPA Academy - Incident Investigation in Aquatic Facilities.

Designed specifically for professionals working across the aquatic and recreation industry, this practical program develops the skills and confidence required to lead structured, evidence-based incident investigations.

Moving beyond the immediate cause

Effective incident investigation is about more than identifying individual actions or immediate causes. It requires looking deeper at the systems, processes, human factors and organisational conditions that may have contributed to an incident.

Through real aquatic incident case studies, investigation reports and practical exercises, participants will explore how to collect and analyse evidence, conduct interviews, identify contributing factors and apply investigation methodologies to uncover root causes.

The course covers key areas including:

  • Incident response and initial investigation actions

  • Securing an incident scene and preserving evidence

  • Evidence collection, documentation and interviews

  • Root cause analysis

  • Human and organisational factors

  • Common investigation pitfalls

  • Developing effective investigation reports

  • Creating practical and actionable recommendations

  • Using investigation findings to strengthen organisational systems and prevent recurrence

Participants will also take part in scenario-based investigation simulations, evidence collection and interview exercises, facilitated workshops and group discussions, providing opportunities to apply their learning to realistic aquatic facility scenarios.

Practical skills for aquatic industry leaders

The program has been developed for professionals who may be responsible for investigating, managing or responding to incidents within aquatic facilities, including Facility Managers, Duty Managers, Team Leaders and Supervisors, WH&S Managers, Asset Managers and Senior Managers.

Participants will leave with practical tools, templates and approaches that can be applied within their own workplace to improve investigation processes and support continuous improvement.
