Understanding the Question Page
Overview
The Question Page is where individual assessment questions are reviewed and updated. Every assessment within Assessment Manager is completed at a question level, making the Question Page the foundation of the platform's continuous improvement approach.
Each Question Page allows users to review the current maturity rating, update the maturity level, add notes and evidence, review improvement suggestions and create actions within an improvement plan.
Because all maturity ratings and dashboards are calculated from individual questions, keeping information on the Question Page current and accurate is essential to ensuring that your assessment reflects the true maturity of your facility.

What Information is Available on the Question Page?
The Question Page includes:
Assessment Question
Current Maturity Rating
Five Maturity Descriptions
Notes and Comments
Supporting Evidence
Improvement Suggestions
Improvement Plan Actions
Together, these areas provide all of the information required to assess, review and continuously improve a particular aspect of your operations.
Step-by-Step Instructions
Step 1: Open an Individual Question
From the Module Dashboard, select the question that you would like to review.
This will open the Question Page and display the current maturity rating and all associated information for that particular question.
Step 2: Review the Current Maturity Rating
At the top of the page, review the current maturity rating that has been selected for the question.
The maturity rating should represent your facility's current systems, processes and practices rather than future intentions or planned improvements. Before making any changes, it is useful to review the existing information and consider whether it still accurately reflects the facility's current position.

Step 3: Review the Maturity Descriptions
Each question includes five pre-written maturity descriptions corresponding to the five maturity levels:
Starting
Improving
Delivering
Maturing
Excelling
Read each description carefully and determine which one most accurately reflects your facility's current practices and evidence.
The maturity descriptions are specific to the assessment type and question being reviewed and are designed to remove ambiguity and provide a consistent approach to assessing maturity.
Step 4: Update the Maturity Rating
If required, select a new maturity level that better reflects the current state of the facility by clicking on the star that best represents your facilities level
Updating the maturity rating will automatically update the maturity ratings for the module and the overall assessment. Because all dashboards are live, changes will be reflected immediately throughout the platform.
Step 5: Add Notes and Comments
Use the notes and comments section to record additional information about the question.
Comments may include why a maturity rating was selected, recent improvements that have been implemented, challenges that still exist or any other information that may assist future reviews.
Good notes and comments provide context and help other users understand the reasoning behind an assessment decision.

Step 6: Add Supporting Evidence
Where appropriate, upload documents, policies, procedures, records, photographs or other evidence that supports the selected maturity rating.
Supporting evidence helps validate the assessment and demonstrates that systems and practices are in place. It also provides a useful reference point when reviewing progress in the future.
Step 7: Review Improvement Suggestions
Each question contains improvement suggestions that are designed to help facilities progress to the next maturity level.
These suggestions provide practical guidance and ideas that can be considered when planning future improvements. They are intended to support decision-making and should be considered alongside your facility's risks, resources and priorities.

Step 8: Add Actions to an Improvement Plan
If a question identifies an opportunity for improvement, actions can be added directly to an improvement plan.
This allows facilities to convert assessment findings into meaningful actions and helps ensure that opportunities for improvement are actively managed and monitored over time.

Step 9: Save Your Changes
Once the maturity rating, comments and evidence have been updated, save the question.
Your changes will automatically update the question, module and overall assessment maturity ratings, ensuring that all dashboards and insights remain current.
Tips and Best Practice
Review questions regularly rather than waiting for an annual review period. Assessment Manager is designed as a living assessment system and works best when information is updated whenever meaningful changes occur.
Always use evidence to support maturity ratings wherever possible. This helps create consistency between assessors and provides confidence that ratings accurately reflect the facility's current position.
Pay particular attention to questions that have not been reviewed within the past 12 months, as the information and evidence may no longer reflect current practices.
When adding actions to an improvement plan, ensure they are practical, achievable and aligned with the goals that have been established for the facility.
