Course Description

Incident Investigation in Aquatic Facilities

Lead structured investigations that uncover root causes, strengthen systems, and prevent incidents across aquatic facilities.

This program develops advanced capability in leading incident investigations within aquatic facilities, drawing on expert experience in investigation methodologies, root cause analysis, and system review. Participants will explore how effective investigations move beyond immediate causes to identify underlying system failures and contributing factors.

Using real incident case studies and investigation reports, participants will gain insight into common investigation pitfalls and best practice approaches. The program emphasises evidence-based investigation techniques, including data collection, interviews, and analysis of human and organisational factors.
