This practical, immersive program equips aquatic leaders, supervisors and safety professionals with the skills to conduct structured, defensible investigations that go beyond surface-level findings. Through real-life aquatic case studies, participants analyse incidents involving supervision failures, environmental hazards, emergency response breakdowns and governance gaps — identifying not just what happened, but why.


Facilitated by Craig Roberts, author of the GSPO and a highly experience aquatic safety expert with countless incident investigations, the course combines the ICAM investigation methodology, human factors analysis and hands-on workshops to build confidence in leading investigations that stand up to scrutiny. Walk away with practical tools, structured templates and the capability to understand the root causes and practical strategies to learn and improve.


Course Overview

Incidents within aquatic facilities can have significant consequences for patrons, staff, contractors and organisations. Effective investigation is critical not only to understand what happened, but to identify why it happened and what can be done to prevent recurrence. This advanced program provides aquatic industry professionals with the knowledge and practical skills required to lead, contribute to and support incident investigations within public swimming pools, aquatic centres, water parks and other aquatic environments.

Participants will explore contemporary incident investigation methodologies and learn how to gather evidence, analyse contributing factors, identify system failures and develop meaningful corrective actions. Through practical case studies drawn from real aquatic incidents, attendees will examine how supervision, facility design, operational procedures, maintenance, communication, emergency response and organisational culture can influence incident outcomes. The program emphasises a systems-based approach to investigation, moving beyond individual blame to understand the broader factors that contribute to incidents and near misses.

The program begins by exploring the role of incident investigation within aquatic risk management systems, including legal obligations, organisational responsibilities and the importance of learning from incidents and near misses. Participants will examine common incident types encountered in aquatic facilities and the challenges associated with investigating dynamic operational environments.

The program then progresses to evidence gathering and investigation planning, focusing on scene management, witness interviewing, documentation review, CCTV analysis and information collection techniques. Participants will learn how to preserve critical evidence and establish a clear understanding of the sequence of events.


Participants will then explore incident analysis methodologies, including causal factor analysis, systems thinking, human factors and organisational influences. Through practical aquatic case studies, attendees will identify failed and missing controls, examine supervision and operational factors, and develop findings that are evidence-based and defensible.

The program concludes with reporting, corrective action development and organisational learning. Participants will learn how to prepare professional investigation reports, communicate recommendations effectively, monitor corrective actions and use incident findings to strengthen safety systems, supervision practices, emergency preparedness and overall aquatic facility operations.
